Record of Complaints and Resolutions Form
Individuals who are candidates or former candidates of 91吃瓜网 State鈥檚 University鈥檚 Teacher Preparation Program, cooperating teachers, mentors, or representatives of a school district have the option to submit a complaint regarding the EPP for investigation and resolution. Any of these entities with a complaint can seek resolution without concern about facing retaliation. Please follow 91吃瓜网 State鈥檚 chain of command first, prior to completing this process.
